Forms

You may obtain forms and applications from the main VA Forms page. In addition, this page contains links to submit certain forms online. If you are having trouble locating a certain form or would like more information, please call us at 1-800-827-1000. The following is a list of the most requested forms:

• VA Form 21-526, Vet's Application for Compensation and/or Pension. Use this form if you are applying for benefits for the first time.
• VA Form 21-686c, Declaration of Status of Dependents. Use this form to report any changes of dependents.
• VA Form 21-4138, Statement in Support of Claim. Use this form to tell us anything concerning your benefits.
• VA Form 21-22, Appointment of Veterans Service Organization as Claimant's Representative. This form is used to appoint a service organization to represent you. (Free of charge)
• VA Form 21-4142, Authorization and Consent to Release Information to the VA. Use this form if you would like us to obtain records on your behalf.
• VA Form 21-8416, Medical Expense Report. Use this for if you receive or are applying for pension to submit your medical expenses to reduce your countable income.
• VA Form 21-0516-1, Improved Pension Eligibility Verification Report. (Veteran With No Children) This form is used if you receive pension and you would like to report your income and you have no dependent children.
• VA Form 21-0517-1, Improved Pension Eligibility Verification Report. (Veteran with Children) This form is used if you receive pension and you would like to report your income and you have children as dependents on your award.
• VA Form 21-0518-1, Improved Pension Eligibility Verification Report. (Surviving Spouse w/no Children) This form is used if you receive death pension and you would like to report your income and you do not have children as dependents on your award.
• VA Form 21-0519-1, Improved Pension Eligibility Verification Report. (Surviving Spouse with Children) This form is used if you receive death pension and you would like to report your income and you have children as dependents on your award.
