Dear Participant:
The Department of Veterans Affairs (VA) appreciates your interest for participating in the guaranteed home loan program.
In order to issue a new Lender Identification Number (LIN), you MUST provide the following information (per VA Pamphlet 26-7, Chapter 1, Section 2):
1. Specimen authorized signature for all officers, underwriters, or other personnel who will sign documents related to VA guaranteed loan activities. Please include printed name, title and signature on the enclosed Lender Information sheet.
2. Complete and signed VAF 26-8812, VA Equal Opportunity Lender Certification. Click on form to download or retrieve from: www.homeloans.va.gov.
3. Certify by signing the statement on the attached the owners, any lending personnel or officers VA or HUD has ever debarred or took other adverse action against.
4. Copy of your applicable State issued Mortgage Broker License or Correspondent Lender License is required for lenders in AL, FL, MS, PR and VI.
5. The enclosed Lender Information Sheet must be completed in its entirety. Any changes affecting this information such as name change, address change etc. must be reported immediately to maintain the accuracy of VA ’ s database.
6. Agents submit a copy of this application along with $100 fee to your sponsor.
$100 check should be a company check and should be payable to: Dept of Veterans Affairs (NOT VA).
All of the above documents are required. Double check all six documents prior to submission. Fax ALL completed documents to 727-319-7763. If applying as an agent forms should be sent to sponsor-not VA.
VA may at its discretion order a credit report and or interview principal officers.
Allow 2 work days from fax submission for receipt of your LIN.
